I mean, totally, the whole medical system is on the verge of collapse due to this embargo. And it's really important that the borders are kept open and remain open even after this crisis and humanitarian efforts that the world are doing are completed, that they allow people to come in from the medical side and educate, train as well as bring in supplies and sort of bring the whole medical infrastructure up to date on how medicine is done. Doctors walking away from hospital\u003cbr/\u003eDoctors from the United States who rushed to the Gaza Strip to help the war wounded quickly learned that their challenge went beyond treating shrapnel injuries.\u003cbr/\u003eThe eight American specialists found themselves operating on patients who had fallen victim to the 20-month-border closure that had crippled Gaza's health care system even before Israel's offensive against Hamas.\u003cbr/\u003eEver since Israel and Egypt closed the borders in response to the Hamas takeover of the territory in June 2007, it's been difficult for Gaza patients to cross the border for medical treatment.\u003cbr/\u003eThe closure also dealt a further blow to Gaza's underdeveloped health care system, which lacks sophisticated equipment and key specialists. \u003cbr/\u003eHospitals often operate on generators because of disrupted power supplies and spare parts for some machines are unavailable.\u003cbr/\u003eOn the eve of the war, Gaza's hospitals had run out of 250 of the basic one thousand health care items and were short on 105 of 480 essential drugs, including some cancer medications and anesthetics, according to the World Health Organisation.\u003cbr/\u003eIn this vulnerable condition, Israel launched its onslaught against Hamas and the Gazan population.\u003cbr/\u003eMore than one thousand and 280 Gazans were killed in the three-week offensive, according to the Palestinian Centre for Human Rights. \u003cbr/\u003eMore than four thousand people were hurt, including about 500 critically. \u003cbr/\u003eThe 600 most difficult cases were allowed passage to Egypt.\u003cbr/\u003eSome of the exhausted Palestinian doctors were finally given relief by foreign medical teams that arrived in Gaza once a parallel ceasefire took hold on January 18. \u003cbr/\u003eThe Americans, including a plastic surgeon and a radiologist, arrived on Sunday, bringing much needed expertise and also medical equipment.\u003cbr/\u003eSince their arrival, they performed more than 15 procedures, including skin grafts and cancer surgery - at times improvising as neither supplies or equipment were sufficient.\u003cbr/\u003eOn Tuesday, the team removed a kidney tumour the size of a honeydew melon from a four-year-old boy in a five-hour emergency surgery at Gaza City's Shifa Hospital.\u003cbr/\u003eThe tumour was advanced and without quick intervention, the child would likely have died, the US team said. \u003cbr/\u003eDoctors in Gaza didn't have the expertise to operate on him and the boy's father had been unable to get him transferred quickly to Israel or Egypt.\u003cbr/\u003eAbdullah's father, a butcher in Gaza City's outdoor market, said his son was diagnosed only a month ago, after his belly kept swelling. \u003cbr/\u003eHe said his request for treatment outside Gaza was still hung up in bureaucracy when the foreign doctors arrived.\u003cbr/\u003eEven after the surgery, Abdullah's prognosis is uncertain. \u003cbr/\u003eHe'll need followup treatment, including advanced chemotherapy or radiation, which are not available in Gaza.\u003cbr/\u003eFor this special treatment, Abdullah would have to go to Israel. \u003cbr/\u003eEven during the 20-month closure, Israel permitted several hundred patients a month - some 900 at its peak - to reach Israeli hospitals for treatment not available in Gaza.\u003cbr/\u003eBut each trip across the heavily fortified Erez crossing into Israel requires a complicated series of permits from officials in Gaza, the West Bank and finally Israel's Shin Bet security service.\u003cbr/\u003eIn recent months, the number of rejections on security grounds increased, according to Physicians for Human Rights in Israel, which helps Gaza patients.\u003cbr/\u003eIt says about one thousand referrals a month are needed, but that in the period before the war, only about half that number were reaching Israel.\u003cbr/\u003eAn Israeli Foreign Ministry spokesman said Gaza's Hamas rulers were responsible for any hardship in Gaza but that Israel had gone out of its way to ensure ongoing medical care.\u003cbr/\u003eHowever, international aid groups say the pre-war trickle of aid shipments is not sufficient to deal with Gaza's growing humanitarian crisis. \u003cbr/\u003eRebuilding homes, factories and several health care centres is estimated to cost about two (B) billion US dollars. \u003cbr/\u003eMany of the wounded will need rehabilitation. \u003cbr/\u003eThe American doctors - who are in Gaza until Friday - have been careful to stay away from politics.\u003cbr/\u003eThe lifting of the closure is linked to complex negotiations between Israel, Hamas, Egypt and others. \u003cbr/\u003eBut Dr. Ahmed Colwell, an
