Instructional Ideas

  • Use this as the next step after a verbal warning to head off management problems before they escalate
  • Keep a file for each pupil and add these forms so that you can track their behavior over a period of time and determine if further action needs to be taken

Classroom Considerations

  • Some of the consequences are fines ($5, $10, $20); if you don't have a class money system set up, you may wish to white out these sections before making copies
  • See the materials tab for two additional behavior-related pages

Pros

  • Provides several sections for students to fill out after they have broken a rule; they must think about their actions
  • Requires a student, teacher, and parent signature

Cons

  • None
